Learning Objectives
- Improve adherence to evidence-based clinical guidelines for geriatric conditions in practice.
- Develop evidence-based plans to diagnose, treat and manage common clinical conditions, chronic diseases and common neurodevelopmental and behavioral health conditions seen in primary care.
- Implement tools to manage chronic care in practice and help patients control chronic illnesses, which may include practice transformation and/or quality improvement activities.
- Demonstrate the ability to communicate effectively with the patient to ensure that diagnosis and treatment recommendations are understood.
- Recognize when to refer to, or consult with, other specialists to provide optimal patient care.
- Utilize strategies to help patients overcome barriers to change in relation to chronic diseases, including cultural competency, literacy skills and access to services or care.
- Educate and counsel patients on behavior modification strategies to reduce risks of complications and improve outcomes related to their neurological and/or behavioral health.
